Patients are asked to bring pertinent medical records, referral forms when required by insurance, relevant test results and a list of current medications and dosages to your initial visit. For your convenience, new patients can download, print and complete forms prior to arrival.
At every visit we ask that you bring your insurance card, photo identification and co-pay. We accept Visa, MasterCard, Discover, checks and cash.
If surgery is required, our staff will work to choose a hospital or surgical center that meets your needs and your insurance plan’s requirements. Should preoperative testing be required, our office will contact you with details.
After-hour issues requiring you to speak with your surgeon can be handled by calling the office at 480.513.2727. Our service will page the physician on call.
Should you need to cancel an appointment, please call the office at least 24 hours in advance.
